open your console,
apt-get remove phpmyadmin
You may remove almost anything in Ubuntu/debian in similar way. (but not ehcp, it is some different)

then, remove /etc/apache2/conf.d/phpmyadmin.conf
restart apache
/etc/init.d/apache2 restart
or
in ehcp gui, sync domains
